What is greater trochanteric pain syndrome (GTPS)?

GTPS is an umbrella term for multiple conditions which cause pain over the bony point on the outside of the hip. It usually occurs due to overloading of the gluteal tendons as they insert around the hip, resulting in inflammation of the tendon (tendonitis) and surrounding tissue.

What causes it?

There may be more than one cause or contributing factor. These can include:

  • Repeated strain or tears of the gluteal tendons
  • Excessive pressure on the gluteal tendons from a tight iliotibial band
  • Running or repeated loading
  • Structural problems of the hip joint
  • Hormonal changes following the menopause
  • Raised body mass index
  • Changes following hip surgery

 

What are the symptoms?

Everyone experiences symptoms differently, but you may notice:

  • Pain over the outside of the hip
  • Pain that may spread into the buttock
  • Pain that is worse when lying on the affected side
  • Progressive pain or discomfort during walking, stairs or activity

 

How is it diagnosed?

Your consultant will examine the hip and discuss your symptoms. Ultrasound or MRI can assess the bursa and tendons, while X-rays may help exclude another cause.
