Sesamoiditis – Pain under the big toe

The sesamoids are two small bones beneath the big toe joint. Sesamoiditis describes pain and irritation around these bones and the tendons that surround them.

What is sesamoiditis?

The sesamoids are two small bones beneath the big toe joint. Sesamoiditis describes pain and irritation around these bones and the tendons that surround them. The sesamoids sit within tendons and help the big toe move smoothly as you walk, run or push off through the foot.

What causes it?

There may be more than one cause or contributing factor. These can include:

  • Repeated loading during walking, running or sport
  • A direct injury
  • A sesamoid stress fracture
  • Turf toe, where the big toe bends back forcefully
  • Wear and tear or arthritis in the big toe joint

What are the symptoms?

Everyone experiences symptoms differently, but you may notice:

  • Pain beneath the big toe joint
  • Pain that comes and goes before becoming more persistent
  • Difficulty continuing with sport or activities that load the forefoot

How is it diagnosed?

Your consultant will examine the foot and may arrange an X-ray. An MRI or CT scan can provide more detail where a fracture or another cause is suspected.

We’ll explain any tests clearly and talk you through what the results mean for you.

How is it treated?

Treatment depends on the cause, the severity of your symptoms and the activities you want to return to. We’ll help you understand your options before you decide what happens next.

  • A boot or specialist insole to take pressure off the area
  • Changes to training or activity while symptoms settle
  • A gradual return to sport
  • Pain relief or anti-inflammatory medicine, if advised
