Iliotibial band (ITB) syndrome

Iliotibial band syndrome, sometimes called runner's knee, causes pain on the outside of the knee. It is often linked to repeated movement and can usually be managed with a programme tailored to your training and recovery goals.

What causes ITB syndrome?

ITB syndrome is most common in people who do prolonged, repetitive exercise such as running or cycling. It may develop after a sudden increase in training or when returning to sport after an injury. Strength, movement patterns, flexibility, footwear and training load can all contribute.

What are the symptoms?

  • aching or sharp pain on the outside of the knee
  • tenderness and, sometimes, swelling
  • pain that worsens during running
  • discomfort when going down stairs or slopes

How is it diagnosed?

Your specialist will discuss your training, examine your hip and knee movement and identify the area of tenderness. An ultrasound or MRI scan may be used if the diagnosis is unclear or another condition needs to be ruled out.

How is it treated?

Most people begin with non-surgical treatment. The aim is to settle the irritation, address the factors contributing to it and build a gradual return to activity.
