Ankle arthritis

The ankle joint sits between the shin bone and the ankle bone in the foot. Healthy cartilage covers the ends of these bones and helps the joint move smoothly. Ankle arthritis develops when this cartilage becomes worn or damaged. The joint surfaces then create more friction, leading to pain, swelling and stiffness.

What is ankle arthritis?

The ankle joint sits between the shin bone and the ankle bone in the foot. Healthy cartilage covers the ends of these bones and helps the joint move smoothly. Ankle arthritis develops when this cartilage becomes worn or damaged. The joint surfaces then create more friction, leading to pain, swelling and stiffness.

What causes it?

There may be more than one reason why this develops. Possible causes or contributing factors include:

  • A previous ankle fracture or severe sprain
  • Gout or arthritis elsewhere in the body
  • A bleeding disorder such as haemophilia
  • A previous infection in the ankle
  • Wear and changes within the joint over time

What are the symptoms?

Everyone experiences symptoms differently, but you may notice:

  • Pain around the ankle, particularly during movement
  • Swelling or warmth around the joint
  • Stiffness and reduced movement
  • Difficulty wearing normal shoes
  • Pain at rest as the condition progresses
  • A change in the shape of the ankle in more severe cases

How is it diagnosed?

Your consultant will examine your ankle, assess its movement and ask how symptoms affect you. X-rays can show changes within the joint. An MRI scan may provide more detail about the cartilage and surrounding tissues.

We’ll explain any tests clearly and talk you through the results, so you understand what they mean for you.

Treatment routes

The right approach will depend on your diagnosis, symptoms, activity level and personal goals.

  • A brace or supportive boot
  • Cushioned or rocker-bottom shoes
  • Pain relief or anti-inflammatory medicine, if advised
  • Steroid injections to reduce pain
  • Changes to activities that place strain on the joint

Your consultant will help you understand which options may be right for you and when your progress should be reviewed.
